Echo images from a dog presented with clinical signs of immune-mediated disease (suspected polyarthritis). Evidence of endocarditis is present on echo, with a large vegetative lesion affecting the aortic valve. An early diastolic murmur was present on exam, caused by severe aortic regurgitation, but the murmur was not noted prior to treatment with steroids (they are notoriously hard to detect).

Here, the endocarditis lesion is causing a degree of both valve stenosis and regurgitation. Severity of AR can be assessed subjectively (colour Doppler shows a signal throughout diastole, with florid retrograde flow visible through to almost the apex) and more quantitatively (a short pressure half-time on continuous wave Doppler, suggesting the ventricle fills rapidly and LV pressure increases significantly during diastole - reducing the pressure gradient and thus velocity of flow. In cases like this, pulse quality is often hyperkinetic because of a low diastolic arterial pressure.

In my experience, endocarditis can be a cause of multi-systemic signs - particularly associated with immune-complex deposition or microthrombosis - before a heart murmur is easily audible and without any obvious signs of congestive heart failure. A focused valve check may be useful in cases that do not fit the typical case appearance for immune-mediated disorders, or for those with a predisposing factor to endocarditis (history of immunosuppressive drug use, known congenital aortic valve disorders, a chronic septic focus or recent long-stay indwelling catheters or central lines).

Although diagnosis would ideally be made using blood cultures, false negatives are common and a combination of typical echo findings, clinical history, presenting signs, and cardiac troponin I concentration can help to identify affected dogs.

This dog initially presented to our colleagues in neurology. Here, episodes of collapse (which sounded cardiovascular to us, rather than neuro) were infrequent but progressively longer and with a slower recovery time. Echo was normal. Although we tried to find evidence of the cause on Holter ECG over several days, we couldnโ€™t prove an arrhythmia. The implantable loop recorder is a helpful tool for cases like this. After setup by a cardiologist, an owner can transmit recorded data from home. The ECG before a collapse episode showed evidence of conduction system disease and long pauses. After review with his owner, we placed a transvenous pacemaker. Clinical signs resolved and the dog was able to exercise normally with a good quality of life. We decided to leave the loop recorder in place to help further diagnostics if required, but since the dog is well itโ€™s become just an interesting radiographic finding thatโ€™s well tolerated by the pet.

Transoesophageal echo (TOE or TEE, depending on your level of British-ness in English!) has become an important part of our interventional and hybrid surgery programme. It helps to improve accuracy of sizing, increase confidence in catheter placement and orientation, and reduce radiation exposure. Real-time 3D imaging via TOE adds another layer of quality to our procedures. In the example here, we can see the anatomy of the patent ductus arteriosus, entering the pulmonary trunk close to the bifurcation, with continuous flow. The orifice is ellipsoid in 3D. Alongside fluoroscopy in theatre, 3D TOE helps us to understand the patientโ€™s anatomy as best we can.

Complex congenital heart disease in a cat presented for investigation of a quiet heart murmur. A complete atrioventricular septal defect (AVSD) is present, with malignant of the interatrial septum, causing double outlet right atrium (DORA) and supra-valvar mitral stenosis (SVMS).

Characteristics of the AVSD to note: a low (primum type) ASD, an inlet VSD, and continuity between the left and right AV valves. The malalignment of the interatrial septum is responsible for not only the shunting of deoxygenated blood into systemic circulation at atrial level, but the creation of a high pressure chamber within the left atrium, into which the pulmonary veins drain. The โ€œtrue left atriumโ€ is very small, above the left AV orifice.

Complex congenital heart disease like this is often associated with pulmonary hypertension and right-to-left shunt, causing erythrocytosis amongst other haematological anomalies.

Surgical approaches are used to treat children, but in cats these pose numerous challenges. Hybrid balloon dilation of the SVMS may help control pulmonary oedema if present, and medical treatment to reduce pulmonary arterial hypertension and control PCV can be useful, and in some cases a pulmonary artery band surgery can help to protect the left heart and pulmonary vascular from over-circulation.
