Here’s how to assess before treating 👇🏼
✔️ Jaw shape & proportions
I look at bigonial width vs. bizygomatic width. If narrowing the lower face will improve balance, it may be appropriate.
✔️ Skin elasticity & tissue support
If there’s already early jowling, laxity, or weak lower-face support, weakening the masseter can actually unmask jowls instead of slimming the face.
✔️ Cause of bulk
Is the fullness muscular (clenching/bruxism) or structural/fat-related? Masseter tox only treats muscle.
✔️ Function first
For TMJ or clenching, I still recommend dental evaluation—Botox alone isn’t always the full solution.
When I do treat, I start conservatively and reassess. More units can always be added—over-treating can’t be taken back.
Bottom line:
If there’s any risk of worsening jowls, I won’t treat this area.
Education > over-injecting. Always.
