We had Hb electrophoresis for a 46 y/o lady with a long history of chronic anemia, and HbH was detected. However, HbA₂ was 2.2%, which was higher than expected for HbH and initially made the diagnosis unclear. We went back and took her history, finding that her previous hemoglobin was 6.5 g/dL and she had received blood transfusions before the test, which explained the results. Golf-ball inclusions on supravital staining helped confirm the HbH diagnosis.

✅ HbH Disease
•Definition:A form of α-thalassemia intermedia caused by loss of 3/4 α-globin genes → excess β-chains → formation of unstable HbH (β₄ tetramers).
•Nickname: Golf Ball Disease → due to characteristic dark, dot-like inclusions in RBCs seen on supravital stain.
✅Genetics
• Normal genotype: αα/αα
• Deletional (80–90%) →—/-α
• Non-deletional (10–20%) → point mutations (Hb Constant Spring) → more severe
✅Pathophysiology
• α-globin deficiency → excess β chains → HbH forms
• HbH is unstable & precipitates in RBCs, forming insoluble inclusions that damage cell membranes.
•Inclusions cause hemolysis (spleen removes damaged RBCs) and oxidative stress, shortening RBC lifespan to 20–30 days (vs. 120 days normal).
✅Clinical Features
• Chronic hemolytic anemia:
• Hb 7–10 g/dL (deletional)
• Hb <7 g/dL (non-deletional)
• Acute crises → triggered by infection, fever, oxidant drugs (esp. HbH Constant Spring)
• Splenomegaly, hepatomegaly, jaundice,gallstones&growth retardation (severe forms)
✅Laboratory Findings
• Smear: microcytosis, hypochromia, anisopoikilocytosis, target cells,tear drop
• Supravital stain: golf ball inclusions in >50% RBCs
• Capillary electrophoresis / HPLC: HbH (5–30%), ↓HbA₂
• Genetic testing: multiplex PCR or sequencing → confirm deletions/mutations
✅Prognosis
• Deletional HbH: near-normal life expectancy.
• Non-deletional HbH: more severe → iron overload, heart failure, pulmonary hypertension, endocrine problems

Peripheral blood shows severe leukocytosis, presence of numerous variable sized lymphoid cells with blastic morphology and frequent smudge cells.
Immunophenotyping shows positive CD2, CD3, CD4, CD25, CD38, and CD56 with Negative CD1a, CD5, CD7, CD10, CD19, CD20 and CD34 expression.
Younger patients with ALCL usually overexpress the ALK protein (90% of children are ALK- positive). The median age of patients who develop ALK-positive ALCL is 35 years and the median age of patients with ALK-negative ALCL is 58 years.
ALK-positive ALCL is characterized by a t(2;5) (p32;q35) translocation involving fusion of the NPM and ALK gene.
