Here’s another exercise breakdown going over a foundational neuro exercise. Resisted walking is an intervention I use very often with patients across many different diagnoses.

I love it because it challenges patients’: endurance, balance, strength and motor control.

There are many reasons why you may choose this as an intervention of choice for your patients based on their impairments and goals, but one of the most valuable in my opinion is because resisted walking is a great way to quickly drive heart rate up and reach the levels of intensity recommended in the literature (moderate to high intensity training is 60-80% of heart rate reserve or 70-85% of maximum heart rate). For this very reason, resisted walking is a great exercise for those with chronic stroke, incomplete spinal cord injury and brain injury because it directly aligns with our Clinical Practice Guideline to improve locomotor function for this patient population (Locomotor CPG, Hornby et al, 2020)

Facilitation techniques I use to help improve the neuromuscular connection and promote increased voluntary activation through purposeful, functional movement. Every repetition is an opportunity to reinforce motor recruitment, coordination, and recovery. ✨ #sci #strokerehab
External cues: visual, auditory or tactile are such an important tool we often use in neuro rehab. Here each cue served to achieve two things: greater amplitude of movement and greater force production.

1️⃣ The resistance band: we often cue people to take larger steps, but them having a visual representation of just how large of movement we want them to achieve replaces internal focused cues (“take a big step”) with an external focused cues (“tap the band with your foot”). Only counting the repetitions that hit the target can make you feel like a drill sergeant as the therapist 😅 however oftentimes with Parkinson’s Disease, movement gets smaller as fatigue sets in. It’s something we may point to our patients that they may not notice themselves, but being able to see their foot not each the target brings awareness to that decrease in amplitude.

2️⃣ Here the cue was auditory as he could not see the punching station behind him, but it make a sound each time he hit the correct part. Because simply taping the station was almost silent, instructing him to elicit the loud sound was an external auditory cue that encourage greater force production.

3️⃣ Here the external cue was the box which was added to increase the depth of the squat.

Starting a new series where I pick an exercise I love and break it down in terms of goals, function and patient population and I could not think of a best first pick than none other than the hero squat. Hoping this is helpful to individuals looking to practice the exercise, student PTs/OTs or even clinicians looking to learn more about neuro rehab.

Movement: the hero squat involves transitioning from a low to a tall kneeling position with or without arm support. Adding some arm support is a great way to make the exercise more accessible (and provide trunk support) while taking away the box and replacing it with some weights is a great way to grade the activity up.

Goal: the exercise aims to strengthen the posterior chain, and particularly the glutes. Depending on the level or type of injury, this exercise can also target trunk control.

Functional application: it can help improve sit to stand and will actually be one of my go to when noticing a lack of terminal hip extension during the sit to stand. This can sometimes be tricky to cue in standing, so by taking away a degree of freedom (feet) and having someone kneeling, the concept of bringing the hips forward often translates more easily.

Patient population: this can be used with different patient population and for various reasons. I particularly love using this exercise with our patients with spinal cord injury and add additional cues for pelvic tilts because this set up allows for either 1) encouraging recruitment of affected muscles or 2) let someone figure out compensations through arm support or upper trunk movements to still achieve the position. This now becomes a functional movement for bed mobility or even physical intimacy and reminds us that compensations are not necessarily a negative thing when they allow someone to do something functional and all other options have already been exhausted.
