⛔You should Remove Any Rust at Initial Visit😎
Metallic (ferrous) ocular foreign bodies begin to oxidize, forming rust at the injury site within just a few hours of becoming embedded in the cornea. The resulting rust usually forms a ring around the metallic foreign body. If left there, this rust ring can cause an immune response, resulting in extended healing time, inflammation, tissue damage and stromal necrosis.Therefore, it is best to remove as much of the rust as possible, as soon as possible.
⚠️⚠️Post Treatment
The removal of a foreign body often leaves a divet in the corneal surface called an erosion. Depending on the size and depth of the erosion, there are options for treatment.
Applying antibiotic eye ointment and an eye patch for 24 hours is most common. Caution is taken to immobilize the eyelids closed with the pressure patch.
In the event, the erosion is non healing, a bandage contact lens may be inserted. This serves two purposes. A bandage lens allows the patient to have some vision while it negates the deleterious impact of the eyelid blink. The blink of an eyelid can dislodge fresh corneal epithelial cells that have migrated into the erosion to heal the cornea
⚠️⚠️After complete epithelial healing we can give weak steroid to halt any corneal opacity secondary to scaring. Specially in visually significant located F.B.as our case 😞

Totally kidding! These are actually vertical breaks in Descemet’s membrane. Swipe to the last photo of the anterior seg OCT in the last photo if you can’t believe it…
These breaks are most comparable to the horizontal Descemet’s break you get from congenital glaucoma (Haab’s Striae). However, vertical breaks are thought to be more related to birth trauma, such as forceps delivery.
This patient reports she’s had this for all of her life.

December’s Image of the Month features a photo of Subretinal Hemorrhage and fluid from Macular Choroidal Neovascularization.
Most choroidal neovascularization, even if hemorrhagic, results in localized subretinal hemorrhage. In more extensive cases such as the one seen in this image, the hemorrhage can be massive and associated with serous retinal detachment. Sometimes there is breakthrough vitreous hemorrhage as well, simulating a rhegmatogenous retinal detachment. In this fundus photograph, the macula is to the left of the optic nerve and dark subretinal hemorrhage obscures over half of the underlying choroid. Serous detachment elevates the temporal and inferior retina. Even with treatment, in this case, the visual prognosis, is unfortunately, very poor.

🔺 5-year-old patient, after blunt eye trauma presents traumatic cataract with partial dislocation to anterior chamber.
🔺It is estimated that congenital cataracts are responsible for 5% to 20% of blindness in children worldwide.
🔺Cataracts in a child can be:
🔸Congenital (present at birth) or acquired (develop as an infant, child, or adolescent).
🔸Unilateral or bilateral
🔺Cataracts may appear in different parts of the lens and range in size from tiny dots to dense clouds. Genetics, metabolic disorders like diabetes, and eye injury can all cause cataracts.
🔺Trauma is a known cause of pediatric cataracts. If there is no known history of trauma to explain an acquired cataract in this age group, investigation must be considered in children who present with other signs suggestive of child abuse.
🔺Regardless of the etiology, prompt treatment of visually significant cataracts is necessary to allow proper development of vision.
🔺In pediatric patients with leukocoria, perform a differential diagnosis with retinoblastoma, PFV, retinopathy of prematurity, chorioretinal colobomas, toxocariasis, Coats disease, vitreous hemorrhage and other retinal tumors.
