In this article, I discuss my clinical perspectives on posterior restorations using flowable composite resins, including the selection of tip diameter and viscosity, the application of SLDP, and the indications for bulk-fill flowable composites and SFRC.

The most challenging aspect of this case was determining the placement of the ridge located between the distobuccal and distolingual cusps.
Based on the remaining tooth structure after cavity preparation, the presence and general course of this ridge could be identified.
The distolingual cusp corresponds to the hypocone, whose degree of development is known to vary widely in natural dentition.
Anthropological classifications describe the hypocone along a continuum from well-developed to reduced or entirely absent, and this framework guided the interpretation of the remaining structure.
In this case, the morphology suggested a moderately developed hypocone, which informed the positioning of the cusp.
In contrast, the position of the distobuccal cusp was largely dictated by the orientation of the buccal groove and the central fossa.
The ridge was ultimately positioned in a manner that separates the distobuccal and distolingual cusps.
However, a morphology in which the ridge does not completely divide these cusps may also occur in natural teeth.
The prominence and continuity of this ridge likely exhibit considerable individual variation, and further observation of natural dentition will be essential for better understanding the full range of this morphological feature.

Mandibular E often resembles a first molar,
but its mesiodistal dimension is slightly longer, which requires precise balance in occlusal anatomy.
In this case, the tooth had a distinct tuberculum sextum (sixth cusp).
By faithfully reproducing this feature, I could achieve:
•A harmonious occlusal pattern
•Natural functional guidance
•Stable morphology with minimal invasion
This restoration aims to preserve the functionality of the long-term-retained primary molar while maintaining esthetics.

A transverse ridge is a ridge on the occlusal surface of a tooth that forms when the triangular ridges of the buccal and lingual cusps come together and connect.
In the mandibular first molar, some people may have the triangular ridges of the mesiobuccal and mesiolingual cusps close together, so they look almost connected.
The shape and whether this ridge is present can vary depending on the tooth, and it’s an important feature to understand the overall shape of the chewing surface.

I had the great honor of speaking at the Luncheon Seminar of the Japanese Society of Conservative Dentistry.
My presentation, titled “A New Approach to Functional Aesthetic Direct Composite Restoration,” focused on how to create a functional and efficient occlusal morphology in direct composite restorations.
Inspired by the occlusal concepts pursued by dental technician Mr. Takase,
I have incorporated into my clinical practice a method that I developed through exploration to reproduce functional occlusion even in direct composite restorations.
Despite limited seating, the room was full with some attendees even standing and I was deeply grateful for all the positive feedback I received afterward.
