Wrong place — the dorsum is compressed all day by footwear. Press over a bony prominence — medial malleolus or tibial shaft.
Wrong pressure — a poke doesn’t move oedema fluid. Press firm for five to ten seconds.
Wrong stop — don’t say “oedema present” and stop. Grade it one to four.
And remember — sudden unilateral oedema is DVT until proven otherwise. Not for grading. For Doppler.
