Although it has gotten better, post TAVR pacemaker rate is still a problem that we encounter, more often in patients who already have underlying conduction disease - particularly an existing RBBB, and more so with self expanding valves.
Here is a schematic to help understand this. This is a heart model showing where exactly a TAVR valve is placed. I have outlined the conduction system that sits adjacent to the implant. Just below the AV node and aortic annulus sits the membranous septum. This is what contains the bundle of HIS which then branches into the left and right bundle. The most likely areas to be irritated by a TAVR implant are the hundle of HIS or the proximal part of the left bundle branch. So you can imagine if someone has a RBBB and we knock out the Left bundle it leads to heart block, or if we simply knock out the HIS it will do the same.
Also - since we are counting on radial force to implant the valve we tend to over size it by 5% up to 20% in some cases to ensure a good seal. The trade off is the force it exerts to maintain a good position can cause conduction system injury.
We have minimized block by trying to implant the valve as high as possible. This will avoid the valve pushing more on the conduction system during implant.
We also have new implant techniques for the Evolut valve known as the "cusp overlap" technique allowing more precise and higher implants that has significantly reduced our pacemaker rate.
Pacemaker rates are down, but probably still remain in the 6-14% range. Still - the trade off the risk of needing a pacemaker to me is still better then an open heart surgery! What do you think?
