If I needed hip surgery tomorrow, here is exactly what I’d look for.🦴👇

As an orthopedic surgeon and sports medicine specialist, I see the “behind the scenes” of recovery every day. If I were the one on the table, I wouldn’t leave my outcome to chance.

Successful hip arthroscopy isn’t just a simple procedure - it’s a specialized process from the first incision to the final milestone.

Here are the 3 non-negotiables I’d insist on for my own care:

1️⃣ The Specialist Factor: I’d ensure my surgeon is fellowship-trained in sports medicine and hip arthroscopy. This isn’t a “general” procedure. High volume = better outcomes. Period. Ask your surgeon about their training, specific experience, and how many hip arthroscopies they perform every year. 📈

2️⃣ The Day 1 Protocol: I’m starting PT on postoperative Day 1. Waiting weeks leads to muscle shutdown and increases your risk for adhesions. You have to wake up those glutes and hip stabilizers immediately, and I begin passive range of motion for all my patients right away. ⚡️

3️⃣ Milestones > Calendar: I don’t care if it’s been 16, 20 or even 24 weeks. I’m not returning to sport until the data says I’m ready. Objective testing for strength and stability is the only way to minimize re-injury risk. 📏

Successful surgery is a team sport between the surgeon, the therapist, and the athlete.

Returning to sport after ACLR is about more than just hitting a 9-month milestone. To slash the risk of reinjury, we look at objective data—not just the calendar.

Here are the 3 metrics that matter most:

✅ Functional Hop Testing: A battery of tests (e.g., single-leg hop for distance, triple hop) that assesses power and landing mechanics with >90% limb symmetry.

🦵Strength Testing: Crucial for stability, requiring 90% or higher strength symmetry between the injured and uninjured limb.

🧠 Psychological readiness: Assessment of confidence and fear of re-injury, typically measured using the ACL-RSI scale.
