Ear molding is a way to gently reshape an infant’s ears while the cartilage is pliable.⁠
Most ears at the top have upper and lower folds that come together to make a curved Y shape with the antihelix.⁠

Here the upper crus is not seen in its usual position. Instead, there is a crus going back instead of up and forwards.⁠

My training involved weekly teaching conferences with many greats in plastic surgery -,, Jeff Kenkel, Fritz Barton, and many more.

Any extra information, any fluff, would expose yourself to lines of uncomfortable questioning, and definite derailing and public dismantling.

Conversely, when prepared, I remember flying through two cases in 6 minutes with Dr. Rohrich, and feeling like we absolutely hit all the high points.

The structure for us:

1. 1-liner summary. No fluff. Every detail leads to obvious questions and conclusions. Anything else was readily available, but not offered.

2. Proactively hit the branch points in decision making. Work up, prep, optimization, etc.

3. Understand what choice you (or really your attending made) for intervention, and why, and why not other options on the table were not taken.

4. A little bit about technique, anatomy, other variations on the surgery. Sometimes to satisfy curiosity about how an attending pulls off a certain surgery (like Dr. Byrd’s endoscopic brow and midface lift), other times to review other historic options that are underused, but deserve revisiting.

5. Understanding common and uncommon but devastating complications, and how to handle those. Facelift skin necrosis, free flap compromise, etc.

6. Sometimes dipping into ethical questions. E.g., unhappy patients wanting a refund.

7. Rinse and repeat, for all of your cases for the week.

Every other specialty and medical school rotation will have their own framework that the attending will expect, and any break in that flow leads to an immediate screeching halt.

Figure out what that expectation is, show you can master that, before you go on and try to riff on it.

Finally, the process is iterative. Write down the 1-liner and key points. As you read and learn more, you realize what additional info needs to be included, and what can be removed.

About 1 in 3 patients with cleft lip and palate benefit from jaw surgery. ⁠

Aligning the upper and lower teeth helps people fully chew their food, and reduces wear and tear on the TMJ's. ⁠

We also consider facial balance and harmony when planning the surgery and final outcome.⁠

Jaw surgery can often be done in a single surgery, sometimes involving just the upper jaw, and sometimes with the lower jaw as well.⁠

Here, the amount of advancement was well over 10mm, which is the point at which we usually move towards distraction over a single surgery.⁠

Distraction involves cutting the bone and freeing up scar tissue, but then gradually moving from the starting position by 1mm per day. ⁠

New bone is deposited in the space made as the upper jaw moves forward. The surrounding blood vessels, lining, and old scar tissue all expand and accommodate the new bony position.⁠

We usually keep the hardware in place (much more comfortably after finishing advancement!) for 3 months while the new bone hardens.⁠

At that point, minor adjustments can be made as needed with additional movement or augmentation, or just removal of the hardware.⁠

Here, we performed some fat grafting to the upper lip, expecting about half of the fat to reabsorb and half to stay long term.⁠

He is seen here at about a month after that surgery -- we look forward to sharing more photos as all the swelling comes down!⁠

Haha, yes, it turns out I cringe hearing myself speak, seeing myself move around, or just thinking about being on camera.

Craniosynostosis is a condition where the sutures (or seams) of the skull close prematurely. The normal growth from the sutures is stopped.⁠

The other growth plates work to keep up with the growth of the brain. But as more sutures are fused, the risk of excessive, harmful pressure increases.⁠

This can affect development and lead to blindness.⁠

This patient has bicoronal synostosis, where two sutures that go across the front of the scalp like a hair band are both fused.⁠

This can lead to a characteristic short skull from front to back. As the other sutures work overtime, this can also lead to a much taller skull.⁠

For this patient, posterior vault distraction was performed. This involves the release of the skull with our neurosurgeons. Immediately some decompression of the brain was seen.⁠

After that, distractors were implanted and used. These provide gradual movement of the bone edges apart. The space in between fills in with bone, and the scalp is gradually stretched and adapted to the increased volume.⁠

After the bone has some time to consolidate, or harden, the hardware is removed.⁠

This condition may be seen without any other medical problems, or may be see as part of a larger constellation of findings, like with #apertsyndrome #crouzonsyndrome #pfeiffersyndrome #antleybixlersyndrome #saethrechotzensyndrome and.⁠

Juggling a few dozen different things every day?

I am a native Texan who studied biomedical engineering. I then did medical school in St. Louis at, with a side stop and some traveling throughout SE Asia and visiting at Chang Gung.
