⏳Delay to diagnosis in axial SpA (axSpA) is longer than in many other rheumatic diseases. Prolonged delay is associate with poorer outcomes, including functional impairment and quality of life. Our aims were to describe global variation in delay to diagnosis, factors associated with delay, and delay compared with PsA.
📖A total of 64 studies reported the mean diagnostic delay in axSpA patients. The pooled mean delay was 6.7 years (95% CI 6.2, 7.2) with high levels of heterogeneity. Delay to diagnosis did not improve over time when stratifying results by year of publication. Studies from high-income countries (defined by the World Bank) reported longer delays than those from middle-income countries. Factors consistently reported to be associated with longer delays were lower education levels, younger age at symptom onset and absence of extra-articular manifestations (EAMs)
👀🚽The key EAMs of axSpA are inflammatory bowel disease (IBD), uveitis, and psoriasis. EAMs carry their own morbidity and often warrant treatment in their own right. Furthermore, the presence of EAMs may influence the choice of therapy used to treat the underlying axSpA. 👀Pain is weird and wonderful. Hurt CAN equal harm! But pain is not always an accurate measure of damage! 🤨
📖A builder spent a week with toothache, without realising he had fired an 8cm nail up his nose. Patrick Lawler thought he had just accidentally banged himself in the face with a nailgun. But, in fact, a nail had gone through his lip and stopped just inside the front part of his brain, missing his right eye by millimetres. Despite suffering blurred vision and toothache, the 23-year-old, who works in a Colorado ski resort, thought no more about it.
😕But when painkillers and ice failed to stop the pain after six days, he went to the dentist where X-rays revealed a nail stretching from teeth to skull.
🙄Mr Lawler said: 'The nailgun recoiled, flipped 180 degrees and shot the nail through the front of my lip.' Once the nail was discovered, he was taken to hospital in Denver. It was removed in a six-hour operation which could have caused blindness or brain injury. Neurosurgeon Sean Markey said: 'This is the second one we've seen where the person didn't actually realise the nail had been embedded in their skull.' But Mr Lawler, who plans to frame the nail and X-rays, said: 'It makes you think about a change of profession. I might use a smaller nailgun in future or maybe I'll just stick to hammers.'

A builder aged 29 came to the accident and emergency department having jumped down on to a 15 cm nail. As the smallest movement of the nail was painful he was sedated with fentanyl and midazolam. The nail was then pulled out from below. When his boot was removed a miraculous cure appeared to have taken place. Despite entering proximal to the steel toecap the nail had penetrated between the toes: the foot was entirely uninjured.
So what is happening here?
Was the pain not real? Pain is always real no matter what.
If the nail hadn't actually gone through the foot then why was pain present? Beliefs and expectations.
Our beliefs can change our behaviour. Sometimes our beliefs might need to change to help make progress. This can take time and is a process which might be uncomfortable at times but working with professionals who you can trust is a key part of the process.

⚠️Treat the person and not just the body part⚠️
👀 In an age where we have become far more fine tuned to the biopsychosocial needs of our patients, it is easy to associate this mainly with those who are suffering with back pain or neck pain, but we really mustn’t neglect our hip pain patients. It’s easy for us to fall into the patter of referring to patients with their pathology, GTPS, FAIs, Adductor pain etc, but we can’t forget that these hips are attached to people!
🙄Whatever the pathology, it’s imperative that we take our time to really listen to our patients. Those with persistent hip issues commonly have been suffering with pain for a long period of time. Whether it’s the fact that they have been suffering with OA and have been waiting a year for a hip replacement, or whether they’re a young athlete currently unable to train, the social and psychological implications is going to be really evident and we mustn’t sell them a disservice by addressing that.
😩Yes, all of the sexy rehab video’s and exercises on IG are great, but I guarantee you nothing is going to help your patient more by sitting down with them, asking them what their main concerns are, what their goals are, and listening to them. Once you’ve done that, you’re already winning.

👀Aims: The purpose of this study was to compare the effectiveness of surgical repair to conservative treatment and subacromial decompression for the treatment of chronic/degenerative tears of the rotator cuff.
📚Materials and methods: PubMed, Cochrane database, and Medline were searched for randomized controlled trials published until March 2018.
🙄Results: Surgical repair resulted in a statistically significant improvement at one year compared with conservative treatment and subacromial decompression alone. In the conservatively treated group, 11.9% of patients eventually crossed over to surgical repair.
😁Conclusion: The results of this review show that surgical repair results in significantly improved outcomes when compared with either conservative treatment or subacromial decompression alone for degenerative rotator cuff tears in older patients.
⚠️However, the magnitude of the difference in outcomes between surgery and conservative treatment may be small and the 'success rate' of conservative treatment may be high, allowing surgeons to be judicious in choosing those patients who are most likely to benefit from surgery.

⚠️Between 2017-2020 there have been at least ten original, independent studies fully peer-reviewed and published in high ranking journals investigating the long-term effects of steroid injections for knee OA. All studies show an association of steroid 💉 use and either worse outcome than comparator or harm in the long-term for knee OA.
👌I appreciate that all ten of these studies have limitations. But I repeat neither any of the ten nor any other studies have shown any benefit of steroid injections in the long-term for knee OA.
📚A systematic review (Geogori et al. 2018) is published with a focus on long-term benefits of knee OA treatments and states unequivocally that steroid injections have no long-term benefit in knee OA.
💉Cortisone for knee arthritis is a medical reversal that needs to happen, and we really need to critically assess not only medical education but also the process for expert consensus statements as to why there is so much resistance to admitting when we’ve got it wrong!
