From April 30 to May 5, I had the honor of participating in one of the most progressive international conferences in facial plastic surgery in Sรฃo Paulo, organized by Lessandro Martins and Thaรญsa Martins
More than 300 participants from all over the world attended the conference, with 25 international speakers and over 40 Brazilian speakers sharing their experience, philosophy, and surgical techniques.
The conference was fully dedicated to Deep Plane Facelift and Endoscopic Facelift surgery and included an extensive lecture program, 10 live surgeries broadcast directly into the main hall, and 2 full days of cadaver lab training.
What makes this conference different is the level of immersion into the philosophy and anatomy of these procedures. Every year, world leaders in facial rejuvenation surgery are invited to share not only techniques, but also their surgical thinking and experience.
At the same time, you become deeply connected with the Brazilian experience in facial plastic surgery - an experience that is both historically and technically unique.
Lessandro pays enormous attention to every detail: organization, service, filming quality, and the educational atmosphere itself. The conference featured a massive 40-meter LED screen, nonstop coffee breaks, beautiful brunches, and even evening food and drinks served directly inside the lecture hall after long surgical sessions.
For the Gala Dinner, Lessandro created an entire temporary restaurant with a professional chefโs kitchen, elegant dining area, and cocktail bar, creating an unforgettable atmosphere for all guests.
This was my fourth Deep Plane Brazil, and honestly, this conference played a huge role in the growth of my professional skills in facial plastic surgery.
I was incredibly happy to see colleagues and friends from all over the world and grateful for the opportunity to share my own experience and knowledge.

I was honored to present my lectures on rhinoplasty and share my own experience, while my friend Fas Arshad presented his work in facial plastic surgery. It was especially meaningful to receive an award together for Best Innovative Technology - a recognition that motivates us to keep pushing boundaries and finding better solutions for our patients.

The first step is working with the bony dorsum. In this case, osteotomy is performed using the Letdown technique with a banana-shaped resection. A controlled portion of bone is removed to create the space needed for lowering and repositioning the dorsal pyramid.
The second step is septal work. Here, the High Strip technique is used, meaning the upper part of the septum is carefully resected. This allows the entire osteocartilaginous pyramid to move downward, helping to straighten the nasal dorsum while preserving smooth and natural lines.
After that, attention moves to the tip. In this case, tip refinement is performed using the Polygon concept, improving support, symmetry, and creating straighter, cleaner, and more refined lines. The nose becomes more elegant, defined, and stable.
The final step is fixation. The dorsum is stabilized using a Columella Strut combined with Skin Fixation, ensuring strong support and long-term predictability of the result.
This technique can be performed both with a closed approach and with an open approach, depending on the anatomy and the surgical goals.
Preservation rhinoplasty is not simply reducing the nose - it is about controlling structure, preserving function, and creating a natural, stable, and beautiful result.

The key principle is structure. The septum should be treated as the main supporting pillar of the nose - the central framework on which the rest of the nose is positioned. When the septum is aligned correctly, the bones and the upper lateral cartilages can be corrected around it in a more stable and anatomical way.
This approach helps create a nose that is not only straighter, but also more natural, more harmonious, and more stable over time. It also reduces the risk of secondary deformities and other long-term complications.
At the same time, the asymmetry of the tip cartilages is corrected, and the tip itself is stabilized in a new position. This is essential because in many cases of a crooked nose, the tip is also deviated or structurally unbalanced.
So the goal is not just to make the nose look straight from the outside. The goal is to rebuild the internal support system correctly, so that both function and aesthetics improve together.

Every crooked nose is different. That is why detailed analysis with CT scan, ultrasound, and 3D planning is so important before surgery.
