What is the difference? These are two different surgical philosophies for changing the shape of the nose. Both can produce beautiful, natural results, and both can be performed through open or closed approaches. The main difference is what happens to the existing anatomy of the nasal dorsum.
1️⃣ THE BASIC PRINCIPLE
STRUCTURAL RHINOPLASTY
The nose is reshaped by separating and modifying its structural components. The surgeon reconstructs the desired shape using bone, cartilage, sutures and, when necessary, grafts.
PRESERVATION RHINOPLASTY
The philosophy is different: preserve as much of the original anatomy as possible and reposition it rather than completely reconstructing it.
2️⃣ WHAT HAPPENS TO THE HUMP?
STRUCTURAL
The dorsal hump is typically removed directly. The bony and cartilaginous dorsum is then reshaped and reconstructed.
PRESERVATION
Instead of cutting the hump away from above, the surgeon works underneath the dorsum and with the septum, allowing the entire dorsal structure to move downward.
3️⃣ BONE–CARTILAGE CONNECTION
STRUCTURAL
The natural continuity between the nasal bones and upper lateral cartilages may be opened and subsequently reconstructed.
PRESERVATION
One of the main goals is to maintain this bone–cartilage connection and preserve the natural dorsal lines.
4️⃣ SEPTUM
STRUCTURAL
The septum may be modified to correct deviation, support the tip or provide cartilage for reconstruction.
PRESERVATION
The septum becomes an important part of lowering the dorsum. By removing a controlled strip of septal cartilage and releasing specific attachments, space is created for the nasal pyramid to descend.
5️⃣ RECONSTRUCTION vs REPOSITIONING
STRUCTURAL = reshape + reconstruct.
PRESERVATION = preserve + reposition.
This is probably the simplest way to understand the difference.
6️⃣ WHICH ONE IS BETTER?
Neither technique is universally “better.”
Preservation rhinoplasty is not appropriate for every nose, and structural techniques remain essential for many complex deformities, significant deviations and revision cases.

Here is how septoplasty with turbinoplasty works:
1️⃣ DEVIATED SEPTUM & SEPTAL SPUR
First, we identify the deviated portion of the nasal septum and the septal spur that are blocking the nasal airway.
2️⃣ ENLARGED INFERIOR TURBINATES
Hypertrophied inferior turbinates can further narrow the nasal passages and reduce airflow.
3️⃣ MUCOSAL ELEVATION
The nasal mucosa is carefully elevated on both sides of the septum to expose the underlying cartilage and bone.
4️⃣ SEPTOPLASTY
The deviated cartilage and bone are corrected or removed, including the septal spur, while preserving the structural support of the nose.
5️⃣ MUCOSAL REPOSITIONING
The mucosa is returned to its anatomical position and carefully secured.
6️⃣ TURBINOPLASTY
The enlarged inferior turbinates are reduced while preserving their important physiological function.
7️⃣ TURBINATE LATERALIZATION
The inferior turbinates are gently moved laterally to create more space inside the nasal airway.
8️⃣ BETTER NASAL BREATHING
The result is a more open nasal passage, improved airflow, and easier breathing through the nose.

1️⃣ENTERING THE DEEP LAYERS
Through small incisions hidden within the hairline, the surgeon enters beneath the fascia and follows the surgical plane into the deeper facial tissues, beneath the SMAS.
2️⃣APPROACHING THE EYE
Dissection continues toward the bony orbital rim. Here, the orbicularis oculi—the muscle surrounding the eye—is identified, and the relevant retaining attachments are carefully released.
3️⃣RELEASING THE MIDFACE
The zygomaticus major, a key muscle of smiling, becomes an important landmark. Retaining ligaments in the cheek, including the area known as McGregor’s patch, are released to allow the tissues to move.

Across the cheekbone region, facial nerve branches are carefully identified where exposed and protected as the surrounding attachments are released.

1 - EYELIDS
We begin with lower blepharoplasty through a transconjunctival approach an incision inside the lower eyelid, without an external skin incision. Excess fat is carefully reduced, while selected fat is repositioned to soften hollows and create a smoother eyelid-to-cheek transition.
For the upper eyelids, excess skin and, when needed, a small amount of fat are removed to refine the eyelid contour.
2 CHIN & NECK
Through a small incision beneath the chin, we address the structures responsible for fullness in this area. Depending on the anatomy, this may include selective fat reduction, adjustment of deeper muscle volume, and suturing the platysma to improve neck definition.
In suitable patients with good skin elasticity, the skin can adapt to the refined contour without additional skin removal.
3 BROWS & MIDFACE
Through small incisions hidden within the hairline, an endoscope provides a magnified view of the tissues.
In the technique shown, the midface is approached in the deep plane beneath the SMAS. Retaining ligaments are carefully released, with attention to identifying and protecting facial nerve branches, allowing the tissues to move before fixation.
Vertical and upward-diagonal vectors help reposition the brows and cheeks while respecting facial proportions.
