Low levels are really common!
Around 3 out of every 10 people in the UK have low vitamin D levels over the autumn and winter. Patients with a history of skin cancer often have low vitamin D levels all year round as they are actively avoiding any sun exposure.
How much to take? The recommendation is 10microgrms (or 400 IU) per day. Don’t take more than 1000 micrograms (or 4000 IU) as taking too much can be harmful.

However, the talk that impacted me the most and will stay with me for the rest of my career was from patient advocate and GP Dr Suze Mellor. This isn’t the first time this topic has been explored. Previous randomised controlled trials have focused on those at high risk of skin cancer and the data has been inconclusive.
This new research is not a randomised controlled trial but analysed data from US veteran medical records retrospectively. Those who took nicotinamide 500 mg twice daily for at least 30 days were reported to have about a 14% lower risk of developing skin cancer overall.
It’s encouraging data, but since this was a retrospective study we can’t prove the benefit. Also, the study population was mostly involved older, male veterans, so we can’t assume the same effect for everyone.

The picture shows the potential routes a substance can take through your SC. The most common route is squeezing 📈 through the spaces between the cells. This space is packed full of lipids (which of course repel water)💦.
The journey doesn’t end there. You have to remember that the SC is only the top layer of the epidermis. If a molecule manages to weave its way between the bricks of the SC it then still has to get through the rest of the epidermis and then permeate into dermis before it even reaches a blood vessel to enter your circulation 🩸. This is not a small feat!
🤔 Factors determining how well a compound travels through the skin include:
⚡️Molecular size, typically quoted to be less than 500 Dalton as an optimal size.
⚡️Its relative fat and water solubility.
⚡️The ‘vehicle’ used in the formulation.
⚡️Duration of exposure: is it a leave on or wash off product?
⚡️Body site: skin thickness and composition varies, just look at the skin on your eyelids vs the skin on your feet.
⚡️The condition of the skin and presence of skin disease.
✅ Final note: skin penetration is not required for a skincare product to be effective. The target site in many cases may be the stratum corneum itself. For example, many moisturisers will work superficially to replenish the skin barrier and that’s an important job. In dermatology we use many prescription topical therapies that are effective and have various targets within the skin.
