This is a very common question.

And the real answer takes longer than 5 minutes to explain. Which is exactly why most women leave their appointments still confused.

The ideal time to begin hormones is within ten years of menopause or less than the age of 60.

Perimenopause begins before menopause and varies based on ethnicity. The SWAN study has been tracking women in midlife and menopause for over 30 years. Most women have never heard of this study and do not realize that their perimenopausal symptom duration is connected to their ethnicity.

ACOG released updated guidance in April 2026, and it changes how most gynecologists should be approaching this symptom. The previous recommendation allowed for transvaginal ultrasound alone, with biopsy reserved for cases where the endometrial lining measured more than 4mm. That approach is no longer sufficient for most patients.

Three things are driving this shift: rising endometrial cancer rates, racial disparities in high-grade cancers that can occur even with a thin lining, and the technical limitations of ultrasound in patients with fibroids or adenomyosis. Ultrasound alone can miss 5 to 12% of cancers at initial evaluation.

If you’re on hormone therapy and experiencing irregular bleeding in the first 6 months, that’s a recognized side effect and usually not cause for alarm. Beyond that window, it needs to be evaluated by someone who can interpret your full clinical picture.

About 90% of endometrial cancer diagnoses present with postmenopausal bleeding. This symptom is never something to wait on.

PCOS is officially renamed PMOS: Polyendocrine Metabolic Ovarian Syndrome. Published last week in The Lancet, backed by 56 global medical organizations and 14 years of research.

The old name pointed doctors toward ovarian cysts, which most women with this condition did not have. This rename matters. And so does getting care that reflects what we now know.

The math your gynecologist never did for you:
Average age of menopause: 51–52. Subtract your ethnicity-specific perimenopause window. That’s when your symptoms may start.

For African American women, that window can be up to 10 years. For South Asian and Hispanic women, nearly 9. For Caucasian women, approximately 7. For Asian women, around 5.

Add in a history of endometriosis or PCOS, and your timeline may shift either earlier or later, respectively.

The timing hypothesis proposes that MHT initiated closer to the onset of menopause (within 10 years or before age 60) may have cardiovascular benefit or at least neutral effects, whereas MHT initiated later (>10 years.

A 2025 WHI secondary analysis confirmed that in women aged 50–59 with vasomotor symptoms, both CEE alone and CEE+MPA had neutral effects on ASCVD whereas women ≥70 had significantly elevated ASCVD risk.

The 2022 NAMS Position Statement states that for women who initiate MHT more than 10 years from menopause onset or who are aged older than 60, the benefit-risk ratio appears less favorable because of greater absolute risks of coronary heart disease, stroke, VTE, and dementia. A 2025 WHI secondary analysis found no clear signal of ASCVD harm in women aged 60–69 with vasomotor symptoms (CEE alone or with CEE+MPA.

JoAnn Manson, a lead WHI investigator, has stated that women in their 60s are “probably reasonable candidates, but it’s a very individual decision.”
