It was such an honour to speak about cyclical vomiting syndrome alongside the ā€œEmperorā€ and ā€œEmpressā€ of Emesis: Prof. Thangam Venkatesan and B. U. K. Li. They co-led the American motility guidelines for CVS, and it was inspiring to learn from people who have shaped so much of what we know.

I met new friends and new collaborators, and it reminded me how lucky I am to do this work. I honestly love my job and I love being (a small) part of academia, especially when it means advocating for people with functional disorders who are frequently unheard and underserved.

Thangam and I talked a lot about ikigai, that idea of finding the meeting point between what you love, what youā€™re good at, and what the world needs. It feels like weā€™ve found ours in neurogastroenterology, and that is something Iā€™m genuinely grateful for.

And yesā€¦ I managed to fit in an Ohio State game in one of the biggest stadiums in the country, and we beat UCLA!!!

Feeling blessed to have been awarded these two prizes (2024 and 2025) by the International Foundation for Gastrointestinal Diseases at DDW2025, San Diego. Faecal incontinence, or accidental bowel leakage, affects 1 in 7 people in the general population. Many women experience accidental bowel leakage after childbirth. It is a cause of significant distress and I see lots of patients with this condition in my motility clinic. The traditional perspective taught in medical school is that accidental bowel leakage is either ā€˜passiveā€™ (involuntary loss of stools without any preceding awareness) or ā€˜urgeā€™ (sensation to defecate precedes accidental bowel leakage). Our unsupervised cluster analysis of patients recruited from five centres across the USA, led by the Mayo Clinic, challenges this classical perspective. Our new classification system, due be published soon, will hopefully help patients around the world access better treatment options. #grateful #braingutaxis

Hello! Iā€™m back! After completing a fellowship in neurogastroenterology (brain-gut axis) in the USA, Iā€™m finally back in the UK. Itā€™s hard to put into words how much this experience has meant to me ā€” itā€™s been a time of growth, learning, and connection, both professionally and personally.

Iā€™m sorry for my radio silence on Instagram. I admit, it was quite peaceful, and I enjoyed spending time focused on my work. But I was encouraged to come back by the thoughtful messages I received from many of you ā€” sharing how my videos on motility disorders and hypermobility helped you feel seen, understood, and supported.

During my time away, I realised just how important it is to continue providing evidence-based information to an underserved community, and Iā€™m grateful for the opportunity to be a small source of support for so many of you.

Iā€™m excited to pick up where I left off, share more, and continue this journey with you all.

Any idea of what my next video should be on?
As a non-inflammatory connective tissue disorder, hEDS is associated with a multitude of different medical problems, such as autonomic dysfunction, chronic pain, myalgic encephalomyelitis etc. People living with hEDS/HSD represent a distinct subgroup of people presenting to gastroenterology clinics who require a highly specific treatment approach which differs from those who are non-hypermobile.

I have heard numerous unfortunate stories of people living with hEDS/HSD being told that their gut symptoms are ā€˜all in the headā€™. However, evidence suggests that dysautonomia (or autonomic dysfunction) may represent the ā€˜missing linkā€™ between hEDS/HSD and gut symptoms, which I explore in this post.

Irritable bowel syndrome (IBS) is one of the commonest gastrointestinal diseases encountered by doctors worldwide. Iā€™ve come across many people who have been labelled as having IBS without them knowing the definition, risk factors, and management. IBS is not just ā€˜bloatingā€™ - it has strict diagnostic criteria set by the ROME Foundation. Moreover, I gather that the general public donā€™t appreciate just how disabling IBS can be - Iā€™ve come across treatment resistant IBS patients referred to tertiary care who arenā€™t able to socialise with friends because their diarrhoea is so frequent. So, with all this in mind, Iā€™ve created this evidence-based video to raise awareness of IBS.

It seems reasonable that people with hypermobile joints would gravitate towards fields where flexibility is important.

This is a relatively old paper (published in 2004) but it piqued my interest in the association between hEDS/HSD and arthralgia (joint pain) in ballet dancers. Aside from joint problems, I wonder how many student and professional ballet dancers with hEDS/HSD suffer from autonomic dysfunction and gut problems. Iā€™m not aware of any study that has examined symptoms, particularly related to the gut, in a large group of student or professional dancers with hEDS/HSD, and this would be important to identify areas for targeted therapy.

I would be interested to learn from student and professional dancers if they wouldnā€™t mind sharingā€¦
