The first step is working with the bony dorsum. In this case, osteotomy is performed using the Letdown technique with a banana-shaped resection. A controlled portion of bone is removed to create the space needed for lowering and repositioning the dorsal pyramid.

The second step is septal work. Here, the High Strip technique is used, meaning the upper part of the septum is carefully resected. This allows the entire osteocartilaginous pyramid to move downward, helping to straighten the nasal dorsum while preserving smooth and natural lines.

After that, attention moves to the tip. In this case, tip refinement is performed using the Polygon concept, improving support, symmetry, and creating straighter, cleaner, and more refined lines. The nose becomes more elegant, defined, and stable.

The final step is fixation. The dorsum is stabilized using a Columella Strut combined with Skin Fixation, ensuring strong support and long-term predictability of the result.

Once repositioned, the structures were stabilized, and the tip was reshaped and symmetrized to create clearer support, better definition, and a more balanced contour.

At the same time, blepharoplasty was performed, which refreshed the eye area and made the overall facial appearance lighter and more rested.

What is especially important in long-term follow-up is that we can see not only the surgical result itself, but also how naturally the face continues to evolve over time. Five years later, the nose remains more balanced and symmetrical, the eyelid area still looks fresh, and the patient herself looks even more beautiful, confident, and harmonious with age.

2 weeks after post-traumatic rhinoplasty โ€“ closed approach ๐Ÿ’†๐Ÿป Photo and Video of the rehabilitation, examination, and technical analysis. A young athletic patient after a sports injury. Years before, he had already undergone septal surgery.
His main concerns are dorsal hump, wide nasal bones, deviation, and impaired breathing.

After comparing his current CT with the scans taken before his very first surgery, and performing a detailed ultrasound assessment and 3D modeling, it became clear that a portion of the septum was missing. Structural support was compromised. That completely changes strategy.

When the primary request is hump reduction and breathing improvement, the technique must solve both aesthetics and function. In this case, I chose a closed approach โ€“ no external incisions, no rib cartilage harvest.

Instead, I performed a high-strip mobilization with controlled displacement of the osteo-cartilaginous pyramid, repositioning it along a central axis.
The nasal tip was stabilized and perifixated to the vaso-segment using a modified TIP SLING component shift.

The result โ€“ visible straightening and narrowing while preserving structural balance.

You can see how the nose looks during the first postoperative week โ€“ swelling is present, which is completely expected.
On ultrasound, postoperative edema is clearly visible โ€“ objective imaging helps us understand what is happening beneath the skin.

The septum was displaced and realigned together with the upper lateral cartilages and the bony part of the nose, allowing precise centralization and restoration of nasal symmetry. This approach follows the Low-strip osteotomy closed rhinoplasty method, where the septum, cartilage framework, and nasal bones are treated as a single structural unit rather than isolated elements. As a result, the nose can be aligned without external incisions, preserving anatomy and stability.

In parallel, the deformity of the nasal tip cartilage was corrected, restoring balance between the dorsum and the tip and ensuring a natural, harmonious contour. Work on the cartilage of the nasal dorsum and nasal bones allowed refinement of both functional airflow and external shape.

At the same time, eyelid surgery was performed. A lower transconjunctival blepharoplasty allowed repositioning of the fat pads without external scars, with additional work on the lower eyelid ligaments to improve support and contour. Upper blepharoplasty was carried out with a skin incision, combined with careful displacement of the fat pads rather than aggressive removal, preserving volume and avoiding a hollowed appearance.

Combining septoplasty, closed rhinoplasty, and upper and lower blepharoplasty in one operation allows a global approach to facial balance. The goal is not just correction, but preservation โ€“ preserving structure, volume, and natural expression, while improving breathing, symmetry, and overall harmony of the face.
