The 7 stage framework for postpartum recognizes that recovery isn’t linear, and that returning to movement, training, sport, and performance are all different stages.

One size does not fit all.

Every athlete’s recovery is unique. Sleep, feeding, pelvic floor health, mental health, healing, energy levels, and life demands all influence the path forward.

The goal isn’t simply getting cleared.

The goal is helping women move through each stage with the support, information, and confidence they deserve.

Because postpartum athletes deserve more than a 6-week check-up.

This one is of particular importance to me having worked in the space of perinatal care and supporting moms and moms to be for the last 15 years.

The unsubstantiated and harmful narratives that surround pregnancy are many.

I have long been on a mission to change this.

We know now the deconditioning in pregnancy sets mom’s up for risk in the postpartum. This risk is bigger for those with higher exercise demands. The application of our decision aid goes well beyond female football – this is big and this makes me so proud.

Our methodological process in terms of how we rigorously developed this aid is mapped out, but importantly our screening process, five stage progression and regression guidance and communication tools for optimal collaboration are all laid out in this open access paper.

Be sure to carefully review the supplementary information as well, as this is where so much of the contextual gold is contained.

Experiencing lumbopelvic pain (LBPP) can negatively impact quality of life and mental health. Many postpartum women will report fear of movement and avoidance behaviour due to pain, which can further impair their quality of life and perpetuate the pain cycle.

➡️This research shows: Various muscular strengthening exercises targeting the trunk muscles during the postnatal period are associated with a reduction in the symptom severity of LBPP and related disability.

➡️muscular strengthening exercises are feasible and effective

➡️Pregnancy-related LBPP will naturally resolve over time without intervention.
However … ✅ Our data demonstrated that exercise has the potential to accelerate the resolution of pain and related disability.
Currently, there are no pregnancy-specific frameworks on sport participation; however, the conducted literature review highlighted the athlete-identified need for an individualised approach to sport participation.

This guidance is bigger than pregnancy.
It changes how athletes plan their pregnancies, train during their pregnancy and supports a long term vision for the career in sport.

It provides practical advice on modalities and training adaptations for the 5 stages of football participation during pregnancy

The FIFA Stepping into play after childbirth decision aid prioritises the player’s voice and shared decision making within a multidisciplinary team is at the heart of it!

In the process of writing the framework we consulted with many pregnant and postpartum players to ensure that it met their needs and felt easy to use and apply to their own personal pregnancy and postpartum return to play.
