• High-quality composite restorations and crowns cannot be placed under ideal conditions.
• The risk of injury to both the child and the dental team increases significantly.
Beyond the clinical limitations, there is also the psychological aspect.
A child’s experience in the dental chair can influence their attitude toward dentistry for years to come. Fear, loss of trust, and dental anxiety can persist long after the appointment is over.
I believe pediatric dentistry should be based on trust, communication, behavior guidance, effective local anesthesia, and, when necessary, sedation or general anesthesia delivered in a controlled and safe environment.

1. When no radiographs are taken before sealant placement.
Ideally, a CBCT scan should be considered in selected cases. Two-dimensional radiographs may fail to reveal pre-eruptive intracoronal resorption, so evaluating the tooth from multiple angles can help identify hidden pathology before sealing the fissures.
2. When rubber dam isolation is not used.
I understand that not every child can tolerate rubber dam placement, especially when a tooth is only partially erupted and local anesthesia is not desirable. However, I perform sealants under rubber dam isolation in 99% of cases. This allows complete control of moisture contamination, prevents additional bacteria from entering the fissures, and ensures optimal bonding of the sealant material.
3. When the sealant layer is too thick.
An excessively thick sealant does not only affect occlusion. It may also contribute to occlusal imbalance, increase the risk of sealant fracture, and compromise long-term retention. Once a sealant chips, it is no longer completely sealed and should be removed and replaced.
That said, even a poorly placed sealant is often better than no sealant at all.
In this child, the sealants were placed at a relatively late age. Nevertheless, they still provided significant protection. Thanks to the sealant, the resulting cavity was much less extensive than it could have been. Without it, the situation might have been far worse.
