So this is one of my most favourite things to discuss. The immensity of what motherhood is has been obscured in this ‘modern’ world.

It is not given the respect. The changes are absolutely incredible. And it is wild to think our species is here due to the ability of the brain to transform. Such a complex organ. And what really blows me away is that it’s not just us humans, similar changes happen in many mammals.

(I had a line in a slide that didn’t make the cut that frogs don’t have these changes! 😂 okay it was a whole slide - not a line! But it’s important.)

Now, we need to know this because while our brain is literally transforming we are being asked to handle more, endure more, ignore more and do more. We expect us to be who we were before the changes. We expect the same relationships, same priorities, same interests.

It’s preposterous actually. Preposterous because our expectations of ourselves are merely reflective of how society has defined motherhood. And that makes me mad. (I have a very well defined amygdala, now after the brain changes form 3 kids. 😂 Not so defined when I was jumping out of planes and with sharks)

We need to be kind to ourselves. That’s not soft, believe me. It’s not indulgent. It’s necessary. To adapt and enmesh with these changes and this new version of ourselves we have to be gentle.

One little note, for those who didn’t birth their babies- fostered or adopted or caring for a baby - please don’t doubt that you will be incredible and adapt physiologically, too.

Always remember that you’re incredible, and I’m so proud of you!

NO, NOT ALWAYS!!! The shape alone does not indicate a problem! Like everything in medicine it must be taken in context!

🔍So what does the shape actually mean!!!
✨The nipple shape after a feed is the physics of feeding made visible.✨

A ‘lipstick’ shaped nipple usually means the nipple experienced uneven compression during the feed. Soft tissue temporarily moulds when pressure is concentrated between the tongue and the hard palate. The nip shape simply reflects the forces it experienced.

And compression, without pain, can happen for different reasons; sometimes babies are still learning coordination. Sometimes they want to stop the milk while they have a burp or BM 💩. sometimes babies apply extra pressure themselves to slow or regulate the milk flow.
❌ It does not automatically mean there is a tongue tie
❌ It does not mean breastfeeding is failing
❌ It does not mean you have done something wrong

If you note a lipstick shaped nip and no pain - reassure yourself if;
✅Feeding is comfortable
✅No cracks, trauma, or persistent soreness

🔍The improtant question is;
IS THERE PAIN/ DAMAGE AND A 💄 SHAPED NIP?
If the nipple comes out lipstick shaped and feeding is painful, it usually means the nipple has been rubbing against the hard palate during the feed. Repeated friction can lead to soreness, damage, and inflammation. This is a mechanical problem and usually improves when the nipple sits towards rhr soft palate and the jaw is optimised.

If there is pain and the💄 shape - we need to modify the latch
✅try to optimise neck extension (look at my principles of latch)
✅get chin deep on the inside of the breast to allow wide gape so nip can reach soft palate

Breastfeeding is a biomechanical process. The nipple shape is simply the body showing us the forces at work during the feed.

This is a very common question! While there are some babies who will have Reflux Disease - they are a small minority! But we understand the infant science it all makes a bit more sense!

So around 40% [1] of babies have physiological reflux, meaning small amounts of milk come back up because the lower oesophageal sphincter (LES), the muscle that keeps feeds in the stomach, is still immature.

This is normal in infancy.

Young babies, especially under 12 weeks, also have small stomach capacity. A 4 to 6 week old baby typically holds around 90 to 130 mL. [2] Spit ups/posets/small vomits can help babies regulate their intake. Babies sometimes take slightly more milk than their stomach comfortably holds, and bringing a small amount back up is one way the body adjusts volume without harm.

🔍The key question is not whether reflux is present but whether it is causing disease or pain.
Reflux becomes reflux disease when it interferes with feeding, growth, or infant wellbeing.

Features consistent with physiological reflux
✅ Small milk spit-ups or possets
✅ Appropriate weight gain (about 150 to 200 g per week)
✅ Feeding effectively
✅ Settles after feeds
✅ No prolonged or intractable crying
✅ Normal examination

Features suggestive of reflux disease AND needing assessment!
❌ Persistent crying associated with feeds
❌ Poor weight gain (less than 150 g per week)
❌ Frequent large-volume vomiting
❌ Blood or green vomit
❌ Recurrent cough or chest infections
❌ Other medical conditions present
Most babies do not need medication. Reflux medications such as PPIs reduce stomach acid, but acid is not usually the cause of normal infant reflux.

It can be uncomfortable to watch your baby spit up or seem unsettled. It can look distressing even when it is normal. But just as babies gain strength and coordination, the smooth muscle in the oesophagus matures too. As the LES strengthens, reflux usually improves. If you are worried, a careful feeding and growth assessment is the best first step.

This is a big one.

A lot of you with variants of normal might read this and feel upset and invalidated as feeding didn’t go as planned. I understand it’s frustrating when someone tells you that something can be done when it didn’t happen for you. I want to acknowledge that and offer an explanation.

We do not have lactation being taught through structured systems. Our field is doing an incredible job but we are plugging holes quickly without indeoth thought. Common practice becomes the norm and with this there is an implication that that norm is based in evidence. When in fact it is based on repeated practice.

Just remember 16- 20% of population have nip diff. That’s someherr between million 665 to 800 million women/AFABs. It’s not rare. And in many other places where they have not lost the societal and community knowledge of breastfeeding the difference is not an issue. What and how we’re teaching is the issue.

So breast engorgement in the post partum causes so much worry and fear - we have never experienced our breasts in this way! So it’s very inprotant to understand what’s normal and how to manage any issues that do arise!

The engorgement of the first days after birth are often a shock because it is the first time our breasts have functioned in this way.

The emotional shock of the breasts swelling in size and feeling after what may have been a difficult pregnancy,difficult birth with no time to process either can really catch you off guard.

Add to that we are in the midst of the HUGE HORMONAL shift while our brains are structurally changing and our uterus is contacting…it is no wonder so many people are surprised/scared/iverwhelmed…

Please don’t feel nervous, many people tolerate it just fine and it’s just another new thing but if you are scared or anxious or nervous understanding what is happening can be helpful.

If you find it uncomfortable - please don’t hestitate to use ice or ibuprofen.

Okay!!! Been a while since I’ve done a detailed post on Mastitis! Tried to make it a little less technical than the one I did 18 months ago!

It’s so important that we get this right and correct the harmful ideas out there. In doing so, we will reduce pain, minimise harm, improve everything - all by simply by understanding the incredible breast mechanics.

The incorrect understanding we have is the reason for so much distress, pain and the early stopping of breastfeeding before a mum/parent wanted to.

Okay, it’s been a while since I’ve introduced on here! One of my good mates asked me 2 days ago - what actually is Breastfeeding medicine!!?! (She’s totally supportive - she’s a marketing hotshot- and thinks my branding of the field is problematic, naturally! 🤣) but she was right I haven’t really explained what I’m about!

So this is a little about the medical side of me, how I’ve arrived here and my intentions. I had no idea I would end up here and am still amazed at how life moves us and positions us.

I often feel overwhelmed and downhearted by how this essential area has been overlooked and I also feel massively like who am I to correct this? But I have been placed in this position with unique set of skills so I just keep going.

I was never the person in medical school that knew what specialty and where and how I wanted to practice. Paediatrics was a natural fit and I love working in the hospitals but I still didn’t really see a vision. Once I started to see what I needed to do, was so nervous because I don’t want anyone to feel judged or guilty or shamed.

It’s been an absolute honour and privilege to support and walk with so many of you at this pivotal time of your lives. I feel so lucky to have such wonderful work!
