「DNAR指示が出たから、点滴も人工呼吸器も止めていい」
——それは重大な誤りです。
DNARとは心停止時にCPRを行わない指示。
それ以外の医療・看護・ケアには一切影響を与えてはならない。
これはAMAガイドライン1991年以来、一貫して強調されてきた原則です。
しかし本邦の現状は——
✅ DNAR指示の正しい解釈を選択した医療者はわずか21.2%
✅ DNAR指示下で人工呼吸器・血液透析が広く差し控えられている
✅ 患者の意思確認なしに医師が独断でDNAR指示を出しているケースが存在する
✅ DNAR指示内容が診療録に記載されていないケースが半数以上
DNARと終末期医療の混同が、患者の権利を静かに侵害している。
「蘇生をしない」という指示は、
「ケアをしない」という指示では決してない。
