Remember this is an “all in” approach because food is truly medicine in this scenario. In the way you wouldn’t skip doses of medication- it’s key to be consistent with the dietary overhaul so we keep the patient safe! This is not an approach I think is necessary for patients without active disease. In scenarios where patients are not in an active disease state we have the luxury of taking things more slowly and a more stepwise approach. Additionally- this is shared decision making. I always always always assess food insecurity and any other barriers that would make these lifestyle changes not feasible. AND some patients for many different reasons prefer medication over lifestyle and that is also a GOOD PLAN! The priority is that the patient is safe.
