In this episode, we speak to our very first international guests, Úna and Andreas, who share their story of navigating their pregnancy and birth choices as expats having their first baby in Portugal. We hear about their experience of initially choosing a private obstetrician based on word of mouth, before changing care providers late in the third trimester once it became clear that there was a misalignment between their birth preferences, the evidence, and the practices of their chosen care provider. Úna and Andreas’ story is one of research, advocacy, intuition and trust. Today we honour midwives near and far who care for birthing women, people and their families. We spend time honouring the midwives in our circle who inspire us, guide us, and have cared for us as birthing mothers, as well as those who we’ve been privileged to witness as they care for others. We send our love in group chats to our fellow midwifery students who are another day, another essay, another crisis of confidence, closer to graduating. We hold space for the families that didn’t receive the care they deserved or needed, and the midwives who feel obstructed from providing the care they wanted for that family. We smile thinking of the families who feel forever changed by the midwife that walked alongside them through their biggest transformation, who will never forget the way that care felt in their body, heart and mind. We think of those in far corners of the world who are birthing in terrifying conditions, and the midwives and birth attendants who are holding their hands through it all. And as the day closes, we hold tight to the belief that next year, on this day, we will be celebrating as midwives ourselves and practising in a role that aligns with our hearts and our passion.

We are very excited to see our names in print in The Practising Midwife Journal Australia last week. In our piece ‘What Homebirth Can Teach Us’, we reflect on our experience attending homebirths (both through publicly funded programs and private midwives) while on clinical placement and through the continuity of care (or ‘follow through’) experience. We paint a picture of the magic we have seen unfold at home - the ‘extraordinary’ in the ‘ordinary’, the trust and informed decision making that is facilitated by the relationship based model of care, and the clinical lessons that observing and working alongside skilled midwives in this space provide.

Our university, both through the approach to teaching, and through arranging clinical placements, has been supportive of us and many others having these formative experiences and for this we are grateful. Not all midwifery students in Australia are so lucky. We hope that with the expansion of homebirth via more publicly funded programs, and what we hope are positive changes to insurance for homebirth, this changes. The ripple effects of this exposure may be felt through all areas of practice, if we gave people the chance to learn in this way.

‘If we are to stay true to the midwifery philosophy of supporting women to view pregnancy and childbearing as normal physiological processes, then we need exposure to it - to see it, to believe in it, and to be able to promote it, both inside and outside the home. While homebirth may not be for every woman, or every midwife, we all have much to learn from the holistic approach of homebirth midwives to clinical care across the continuum, as well as from the behaviours of a woman birthing her baby physiologically, in her own environment, with her chosen supports.’ (Godfree & Hansen, 2025, p. 31).
2 years ago today - the fondest of memories setting up our birth space, affirmations that I held onto for a long time after birth too. A reminder that birth affirmations are not just for the day or days of your labour and birth, but can play a meaningful role in supporting your mindset as you traverse those more vulnerable final days and weeks of pregnancy. They are also not just for home birth - setting up your labour space for home is just as important even if you are birthing in hospital, knowing that you will be likely labouring at home for a significant portion of time. Have affirmations up around the house, either portable to move with you to move into your chosen birth place or another copy to take there.

Choosing what affirmations, visuals and language speaks to you and where you are at is important too - not just what Google or Instagram will list for you.

Ideally they are written by you for you, and for your story. I know a big focus for me and my second birth, in light of the physical and emotional effects (and therefore prep required) following my first, was on releasing pelvic tension and letting my body and breath do the work. my affirmations reflected this — ”breathe my baby down”, “I relax, I release, I open”, “loose lips”, and the like. But I hardly noticed them in labour, as by that stage the kind of physical and emotional letting go I wanted to promote had become instinctive.

I enjoyed having them up on the wall in those final weeks, allowing me to visualise that dream birth, and was very reluctant to take them down, holding on emotionally to all parts of the birth I was so proud of ✨

Happy New Year!

We’re back from holidays, including a week camping up in NSW with the two birthchoices families and our six kids. It was a rare opportunity to be together in person (not just speaking all day via voice memos) for 7 nights. Elysia cemented her status as the ultimate ‘snack bitch’, and between endless requests for said snacks and generalised chaos (see second photo), we managed some adult conversations and exciting planning for the year ahead.

It’s going to be a huge one for us personally, and we hope for birthchoices. We’re excited to:

✨ Get back into the swing of the podcast, bringing you some more birth stories and highlighting some key choices available to you in pregnancy, birth (and more postpartum than we’ve touched on yet)

✨ Finish our midwifery degrees and (hopefully!) be registered by the end of the year (surreal.)

✨ Launch some exciting new aspects of birthchoices, including bringing you together for some community events, and work on education resources.

Here is a snapshot of some of the tests/screening tools/interventions that may be offered to you in pregnancy.

They are plotted like this to give you an idea of what may be coming up, not because it is the road you necessarily need to travel. Every one of these is a choice for you, ideally to make in an informed way, with full support from your chosen care provider to assess the benefits/risks/alternatives before deciding if it’s a full body yes for you. Some of these are more “standard” than others (a growth scan for example, should never be “standard”, but is something that frequently arises, and more commonly in pregnancies with certain “risk” profiles/certain models of care).

It can be easier to follow the standard path, and certainly you may face resistance in deviating from it, but that shouldn’t be what dictates your decisions. I know for me (Alice), even choosing not to do a dating scan (and also delaying the NIPS I had elected to do) until my 13 week scan, felt like hard work (much confusion on the part of medical receptionists when the system expected me to have ticked certain, very much optional!), boxes. For me, having had a previous loss after a dating scan, I knew it wouldn’t be helpful for me, and wouldn’t change the course of my care. Make decisions based on what is right for YOU, and find a care provider (and/or alternative supports) who backs you in that.

These gestation markers are a guide only. A number of these tools are offered at various/variable points in pregnancy.

This list isn’t exhaustive (there are other tests and screening tools offered particularly in early pregnancy), and some choices around interventions in late pregnancy geared towards induction, that we will address separately. As always, discuss the timing and appropriateness for you with your pregnancy care provider.
