Working the catwalk in Miami!
Showed the world how I use an AI avatar for patient education in my practice.
What started as a different way to think about UCL reconstruction became a technique we later followed through published outcomes.

In this study, we looked at a double suspensory fixation technique for Tommy John reconstruction using a single tunnel in the ulna and humerus with button fixation on both sides. The goal was to create a stronger, more reproducible construct while avoiding some of the issues that can come with multiple tunnels and less secure fixation.

The outcomes were encouraging: significant improvement in functional scores, an average return to play at 9 months, only 1 revision surgery, and no postoperative ulnar nerve symptoms in this series.

The value is not just in developing a new technique. It is in following the athletes, studying the results, and seeing the concept hold up over time.

Dual plating for clavicle fractures is Sexy AF 😜

When managing displaced midshaft clavicle fractures, construct stability matters. Traditional single plating has been effective, but dual plating (typically superior + anterior mini-fragment plates) offers several key benefits supported in the literature:

• Improved biomechanical stability
Dual plating distributes load across two planes, enhancing resistance to torsional and bending forces, especially important in comminuted or high-demand patients.

• Lower reoperation rates
Recent data (including Dual Versus Single Plate Fixation of Displaced Midshaft Clavicle Fractures: A Cost-Effectiveness Analysis, JBJS 2023) suggests fewer secondary surgeries compared to single plate constructs, despite higher upfront implant cost.

• Lower profile implants → less hardware irritation
Using smaller plates reduces prominence, which can translate to decreased symptomatic hardware and lower rates of removal.

• Better for early motion & return to activity
More stable fixation allows for confident early rehab—critical for athletes and active patients.

• Cost-effective in the long run
Even with higher initial cost, reduced reoperations and complications make dual plating a high-value strategy.

This is a great example of how evolving surgical techniques are improving outcomes. Not just healing fractures, but optimizing function and minimizing complications.
